Patient Name — lists all patient names tied to subscriber benefit
Relationship — lists the relationship of patients to member: subscriber, spouse, or dependent
Eligible — the status of the patient's eligibility:
Active — the patient is currently eligible
Inactive — patient is not currently eligible
Proposed — eligibility will need to be confirmed with Confirmed Service
